Summer 2008 Travel Policies & Terms

I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the USYSA, MYSA, Plymouth Soccer Association (PSA), City of Plymouth and their affiliates.  Recognizing the possibility of physical injury associated with soccer and in consideration for the above named organizations accepting the registrant for the 2008 PSA Developmental/ Traveling soccer program and subsequent team activities, I hereby release, discharge and/or otherwise indemnify these organizations, affiliated organizations and sponsors, their employees, volunteers, officers and associated personnel, including the owners of the fields and facilities utilized for the program, against my claim by or on behalf of the registrant as a result of the registrant’s participation in the program.  By agreeing with these terms, the players and the parents or guardians agree to abide by the Plymouth Soccer Association’s player and parent creeds.

Notice: Submission of this form does not guarantee placement on a team.  Traveling soccer is more competitive than recreational soccer.  It involves greater time and travel commitment to the team as players are involved in more competitive games.  Once a player is placed on a team, there will be no refunds of the registration fee.  Your agreement indicates your understanding and agreement to abide by this commitment level and refund policy.
